How to Register

Defined Practice Area
Newry City Centre and 6 mile radius from the practice. If you are unsure if you fall under our catchment area please contact our receptionists who can easily check this for you.
To ensure that your registration is promptly approved please complete all parts of the forms accurately and provide the correct additional documents if required (refer to your registration pack for required supporting documents).
Newborn: Submit form HS123 (obtained from local City Hall when registering the birth) or complete form HSCR1 and registration pack. Refer to registration pack for required supporting documents.
Previously NHS Registered: Submit medical card and registration pack. If you do not have a medical card complete form HS200 and registration pack. If it has been over 1 year since your last registration then you must complete form HSCR1 and registration pack. Refer to registration pack for required supporting documents.
Not NHS registered from EU: Complete form HSCR1 and registration pack. Refer to registration pack for required supporting documents.
Not NHS registered from overseas (non EU country): Complete form HSCR1 and registration pack. Refer to registration pack for required supporting documents including residency permit.
Registration forms to join the practice are available to collect from reception between 9am - 4.45pm each day (and up to 1pm on Wednedsay's)
Children registering under the age of 7 years should provide a copy of their vaccination record along with the standard registration forms.
Republic of Ireland Residents
Note: Only patients living or working in Northern Ireland may register as NHS patients.
If you subsequently become resident outside of the UK (G.B and N.I) you are no longer eligible to be treated on the NHS. You must be living in the country for at least 6 months prior to registration.
If you live in the Rep.of Ireland and work in Northern Ireland then please complete form HSC/R1
